Roster or Sign-in sheet. Suggested for use in conjunction with the scoresheets above. If the archers are required to sign-in then you are much more likely to have all of their data right.

Marshal's Report Form

All Missile Marshals are required to submit the online form at least each quarter.

Event Report Form

Event reports must be submitted by the Marshal In Charge within 30 days of an event.

Hopefully you will never need it.
To be used any time that there is an archery or combat related injury requiring a doctor's care. Notification of the Earl Marshal is required within 48 hours. Copies of this form must be on file within 30 days to the Local, Regional, and Kingdom Archery Marshals as well as to the Local Knight's Marshal, Regional, and Kingdom Earl Marshal.

Consent to Participate and Release Liability in Adobe Acrobat format.

The standard roster waiver used for non-members or those members who can not show a signed Blue membership card to prove that they have a waiver on file. Used at the Gate for events, at marshallate practices, etc.

Minor’s Consent to Participate and Hold Harmless Agreement in Adobe Acrobat format.

Parents must sign these for non-member minors or those minors that can not show a signed Blue membership card to prove that they have a waiver on file. Used at the Gate for events, at marshallate practices, etc. One waiver executed per child.

Minors Permission Form/Event Waiver in Adobe Acrobat format.

The waiver and consent form that must be used when a minor attends an event without their parent or guardian. This waiver must be executed in advance and witnessed by a notary. One form should be executed for each child.
